What Factors Cause Fuel Mileage Problems?

I would like to make myself a check-list of all possible factors which could cause bad fuel mileage. I would prefer answers only from those people who have legitimate mechanical knowledge and understanding about vehicles. (i.e. trained mechanics) Any suggestions excluding environmental causes such as weather, road conditions, speed, grade, etc., would be very much appreciated.

I would sure like to solve this problem, but convetional methods have failed so far. So I would like to make a check list of all possible causes and then go through them one by one and check off what I have done and hasn't worked, then move on to try the next logical option until I get this sorted out.

-Dont floor it when accelerating

-Higher octane gas gets better mileage

-Keep your oil changed every 3,000 miles

-make sure all filters are clean

-consider buying a cold air intake for your vehicle 1-3MPG better than what you have now

-use cruise control if you haev it

-use overdrive instead of drive

-try not using the air conditioning. (I personally never use it in MD...I just roll down the windows) Makes a big difference for my jeep

-Car tune up...make sure everything is good to go

the basics, as rolling resistance (tire pressure) should

FYI-Newer cars (90's on) do better with the windows UP.The a/c doesn't rob the engine of power the way they used to. Don't drive with the windows down-it'l cost you gas milage.
